A comparative study of the potential and revealed accessibility of medical facilities considering the medical travel behaviour

  • Abstract:     Objectives:     Accessibility research of medical facilities is critical method to analyze the convenience of regional access to medical services. Existing research mainly focuses on the potential accessibility (PA) research, and few studies research the revealed accessibility (RA) based on the actual use of medical facilities. The purpose of this paper is to explore a method for calculating the RA of medical facilities based on user travel behaviour to analyze the degree of mismatch with traditional PA and evaluate the accuracy of traditional PA. It also provides insights for accessibility study of other facilities.     Method:     Takes the main urban area of Chengdu as the research area, and based on Didi travel data, the three-step floating catchment area model (3SFCA) is improved by incorporating the actual selection weight, the comprehensive service capacity of medical facilities and fitting the actual medical travel distance decay function to calculate the RA. Then, we compare it with the PA calculated based on service capacity of medical facilities and potential medical needs to analyze the degree of mismatch between the them and identify the types of community access to medical facilities, and research the spatial distribution of them. Finally, we explore the reasons for the spatial differentiation of the two communities.     Result:     The PA of medical facilities is mainly affected by the travel time, and the difference between the central area and the peripheral area is large, but the difference between different districts is not obvious; the RA is mainly affected by the number of medical trips, and the difference between each district is obvious; 182 community with “RA>PA” are mainly distributed in Chenghua and Jinniu District, and 243 community with “RA<PA” are mainly distributed in Wuhou District.     Conclusion:     The results of correlation analysis show that the spatial differentiation of the two types of communities is mainly affected by the RA. Compared with the traditional potential accessibility research, the RA calculated in this study is easier to find the actual differences in the use of medical facilities in different regions, judge the accuracy of the PA, and help relevant departments to find problems in the layout of medical facilities.
